We would like to invite you to take part in our two part study. Upon your completion of the second survey, you will be entered into a prize draw for the chance to win a £25 Amazon gift voucher. Before you proceed we would like you to understand why the research is being done and what it would involve for you. Please read the information sheet which follows and if you wish, talk to others about the study before making your decision . Any questions regarding this study should be directed to the primary investigator as detailed below.

Study information sheet 

This study aims to examine the association between your attitudes and beliefs and your eating behaviour and is part of a doctoral student research project supported by Loughborough University. Approximately 4 weeks after its completion, we will ask you to complete it for a second time. It is expected to take approximately 15-20 minutes to complete on both occassions and should only be completed by individuals over the age of 18 years. 

You will be asked to confirm your wish to participate in the informed consent form below. If at any time, during or after your survey completion you wish to withdraw from the study for any reason, please just contact the primary investigator - Rachel Sharp. You will not be asked to explain your reasons for withdrawing. You will need to supply the email address you provide at the beginning of the survey in order to locate and withdraw your data.

Data Protection Privacy Notice 

Loughborough University will be using information/data from you in order to undertake this study and will act as the data controller for this study. This means that the University is responsible for looking after your information and using it properly. 

What personal information will be collected from me and how will it be used? 

The personal information being collected as part of the study includes your email address which will be used to contacted you in order to ask you to complete the follow up survey. Your age, gender and health information will also be collected. This information will be used to identify relationships between the survey responses and the issues that the study is exploring. This information will be analysed as part of the studies results.  

What is the legal basis for processing my personal information? 

Personal data will be processed on the public task basis. Under the General Data Protection Regulation (GDPR), some of the personal data which will be collected from you is categorised as “sensitive data, to include your email address and the health data collected”. The processing of this data is necessary for scientific research in accordance with safeguards. This means that study has gone through an ethical committee to ensure that the appropriate safeguards are put in place with respect to the use of your personal data.

How long will my personal information be retained? 

This study will not collect or retain any personally identifiable information about you. The data collected will only be retained for the length of time its is required to complete the study.

Will my personal information be shared with others? 

Your data will not be shared beyond the research team (Dr Ian Taylor, Dr Clare Holley and Rachel Sharp).   

Will my taking part in this study be kept confidential? 

All information submitted will be anonymous as you are requested to provide an email address as opposed to a name when completing your survey. The responses are collected within a secure survey tool and downloaded into secure university data storage which is password protected and only accessible for review and analysis by the research team via password protected computers.  

How will the anonymised data/results collected from me be used? 

The data will be analysed and will form part of a doctoral study report and thesis. The results of the study may go on to be published in journals and or shared at exhibitions and conferences.  

How long will the anonymised data/results be retained? 

The data will be retained for a maximum of 6 years and will only be accessed following initial use for clarifications and follow up study or presentation.

Informed consent form 

I understand that this study is designed to further scientific knowledge and that all procedures have been approved by the Loughborough University Ethics Approvals (Human Participants) Sub-Committee.    

I have had an opportunity to ask questions about my participation. 

I understand that taking part in the project will involve responding to questions in this survey.

I understand that the personal information collected will be age and gender. 

I understand that sensitive personal information of my email address and my Body Mass Index (BMI), will be collected during this study.   

I understand that I am under no obligation to take part in the study, have the right to withdraw from this study at any stage for any reason, and will not be required to explain my reasons for withdrawing. 

Use of Information    

I understand that all the personal information I provide will be processed in accordance with data protection legislation on the public task basis and will be treated in strict confidence unless (under the statutory obligations of the agencies which the researchers are working with), it is judged that confidentiality will have to be breached for the safety of the participant or others or for audit by regulatory authorities.  

I understand that information I provide will be used for research outputs, publications, reports, exhibitions and conferences. 

I understand that any personal information collected about me will not be shared beyond the study team.           

I agree that information I provide can be quoted anonymously in research outputs.
